B string buzz

Up and down the fret board. Some frets are dead. Stretched and intonated.
Help!

how high is the action?
truss rod: how much relief?
is the string seated in the saddle properly?
is the string rubbing on something else, e.g. pickup?
look down the neck to see if there is warping

Whats the bridge? if its with multiple new strings, all up and down the neck, I would say some thing to do with the string setting in the saddle.

OFR, just the B - It's really bad from the 12th fret up. (And the D but not as bad.)
