My guess: 1987-89 Charvel Model 2 with unoriginal locknut pressure pads, added second control pot/knob (the metal knobs themselves are not original), added pickup selector switch, two added mini-switches, added neck humbucker, and added middle single coil.

Your ivory white finish has also aged very nicely.

I'd also guess a Japanese Model 2 with nothing original on it except for the JT6 trem system and the volume knob.

as I look at the neck pickup, based on the screw holes, it looks like the original pickup was replaced, the same as the bridge pickup.
But, did the Model 2 come with a neck pickup... specifically a neck pickup that used a pickup ring.

Take the neck off and check the neck pocket or heel for a date stamps.

The Model Series guitars don't have date stamps on the neck or in the neck pocket. My Model 1C doesn't. And a quick search on Google Images for other bolt-neck Model Series guitars tells me the same thing.
