Kahler Quick Release Locking Nut

Hello, newbie here...
I've got a Charvel Model 4 with a Kahler installed and really like it. Only problem is with the nut. Its got a quick release locking nut with the three plastic flip catches. The thing that is puzzling me, is that the flip catches are loose. I've never used one of these before so please excuse me sounding like an arse but can you tell me what is causing this. There are three screws missing (the holes are there but no screws so I am assuming this is part of the problem).

I installed one on my Model 2 and I can tell that you definitely need the 3 missing screws. You can get them from Kahler here: http://www.kahlerparts.com

Although they are now discontinued you can still buy replacement parts for these locking nuts.
