Model 4 input wiring?

Hey guys
I've got this project model 4 that I've been trying to get playable again, but all the wires have come loose from the input socket.. There are two black wires and one white. Can anybody tell me to which point on the input these are meant to be soldered to?

Output. Output. Output. Guitar Inputs are the pickups. Signal goes from the guitar's OUTPUT to the amp's INPUT, not the other way around.

The black wires should go to the jack's ground (sleeve), the white goes to the hot (tip).

The model 4 is active. One black wire is the output ground (this will be coming from the back of a pot and goes to the ring of the jack. The other black wire goes to the sleeve of the jack to turn on the battery. The white wire goes to the tip of the jack.
